For a launch carrying only your payload. The FAA bills the launch operator under 51 U.S.C. 50924, on the payload weight of the whole launch. Not added to the price above.
Not included: insurance, payload integration, transport, range services and spacecraft licensing (FCC, NOAA, ITU). Providers and insurers quote these per mission.
A vehicle appears here only if its price has a source you can open: SpaceX's published Falcon 9 and rideshare rates, the last Falcon Heavy price SpaceX published, Rocket Lab's average revenue per launch, a price Rocket Lab has stated for Neutron, and a press estimate for New Glenn. ULA, Arianespace, ISRO, MHI, CASC and Roscosmos publish no prices, so their rockets are not priced here. Capacities come from our launch vehicle catalogue; $/kg is the price divided by capacity. The FAA fee is computed from the schedule in 51 U.S.C. 50924. Ask providers for quotes.
